qualify an individual to be able to receive hospice and
palliative care services.

- Neurological disorders
- Respiratory disease
- Congestive heart failure
- Chronic obstructive pulmonary disease
- Parkinson’s disease
- Renal disease
- Liver disease
- Alzheimer’s
- AIDS
- General decline from the normal aging process
Pursuant to Title VI of the Civil Rights Act of 1964, Section 504
of the Rehabilitation Act of 1973 and the Age Discrimination act of 1975,
